Required Documents

To receive the CERTIFIED badge, submit the documents below that match the services on your profile. The badge means Ontario’s required credentials are complete.

Required documents by service type
Service Provider	Required Qualifications
Criminal Background Check or Student ID	Every caregiver must have one when providing services. Issued within the past 12 months in Canada. Remove password protection or provide a screenshot of the certificate.
CPR / First Aid	Mandatory for personal support workers and nurses. Must be issued by a Canadian authorized provider with completed in-person training. Online-only certifications are not accepted.
Home Support Services	Home Support Worker certificate, home care assistant certification, or relevant work experience.
Personal Support Services	One of: Personal Support Worker certificate or equivalent (e.g. Developmental Service Worker, Health Care Aide, RPN, RN) from a recognized Canadian institution; proof of at least one year of nursing studies in Canada; or foreign nursing credentials from an officially authorized organization (Ontario registration may still be required to practise as a nurse).
Nurses	Valid CNO registration (or equivalent authorized Canadian nursing registration).
Other Service Providers	Government-issued ID or utility bill confirming your name and address. If applicable, proof of business address. Any other relevant certifications, permits, licences, or diplomas required by regulation.
