Candidate Form

Please ensure that you are legally allowed to work in Canada before completing the Candidate Form below.

The following are the eligibility requirements for the different Home care categories:

  1. Nurses and Therapists: A copy of certificate issued by an authorized organization in Canada, which can be verified online.
  2. Personal Support Worker: one of these certificates:
    • A copy of a Personal Support Worker certificate or equivalent (DSW, HCA, RPN, RN, etc.) issued by an authorized organization or school in Canada.
    • Nursing students who have completed at least one year of studies in Canada.
    • A foreign nurse certificate issued by an authorized organization.
  1. Valid CPR/First Aid certificate is required for Personal Support Worker, Nurses, and Therapists.
  2. Home support services: HSW certificate or relevant work experience.

All service providers are required one of these documents when providing services to residents:

  1. A criminal background checks issued within 6 months in Canada.
  2. Student identification or examination record or Transcript issued by the school attended in Canada.

The CPR/First Aid certificate must: 1. Be issued by a Canadian authorized organization; 2. Have passed all on-site training and examinations. Any certificate that consists solely of online CPR/First Aid training and courses will not be accepted.
