HSCPOA registered NACC certified support worker with dementia care and medication administration training

I am a Personal Support Worker with a PSW diploma, registered with Ontario’s oversight authority for support providers and certified by passing the NACC examination. My clinical placement gave me three hundred supervised hours in a retirement home, caring for residents living with dementia at different types and stages, each with their own daily care plan.

I also volunteer in a long-term care home, supporting residents living with dementia through conversation, games and activities that encourage choice and participation, and I worked privately in a family home for nearly a year and a half with an older couple, one of whom lives with early-stage Alzheimer’s disease. Earlier insurance work with older adults taught me patient communication, accurate records and strict confidentiality.

My skills include help with bathing, dressing, grooming, oral care, toileting and continence care, and with meals and hydration following dietary directions and swallowing precautions. I perform mechanical and two-person transfers, support ambulation and repositioning, record vital signs, document care daily, and report changes in condition to the nurse in charge, following infection prevention and PPE routines.

I hold Standard First Aid and CPR/AED Level C with BLS, Gentle Persuasive Approaches, nonviolent crisis intervention, Montessori-based dementia care, palliative care training and medication administration for unregulated care providers. I work in Persian (Farsi) and English, have my own vehicle, and I am available immediately for day and night shifts, weekends and holidays.
